Terms of Agreement for the use of the SAFY Risk of Disruption Inventory (SRDI) The SAFY Risk of Disruption Inventory is a copyrighted assessment
that predicts outcomes in out of home placement. The instrument predicts youth who are at risk for Disruption during Placement, Negative Discharge
Disposition, and/or failure to Attain Treatment Goals. Specialized Alternatives for Families and Youth (SAFY) of America, Inc. is making the
instrument available to the public domain under the following terms and conditions:
1. The user agrees to utilize the SRDI to predict youth in out of home placement who are at risk of Disruption during Placement, Negative Discharge Disposition, and failure to Attain Treatment Goals.
2. The user shall not utilize the SRDI in a different manner nor in any way add to, edit or modify the instrument without first obtaining specific written permission from SAFY.
3. The SAFY Risk of Disruption shall not be used for research purposes without specific written permission from SAFY.
4. The user acknowledges that Specialized Alternatives for Families and Youth has the sole and exclusive right to license the use and distribution of the SRDI. The user agrees not to provide the SRDI to any other agency, person or organization.
5. The user agrees that any printed or reprinted copy of the SRDI shall contain the copyright symbol, year, and name SAFY as the owner.
6. The user understands that decisions about children or youth in out of home care cannot be made solely on the basis of the SRDI. It is essential that multiple sources of information and fullest knowledge of children and their circumstances be employed when making important decisions about their lives.
